That little "dent" is normal. The firing pin on the AR rifle is free floating ( no return spring) and inertia as the bolt slams shut causes it to lightly strike the primer but never enough to make it fire. Take the bolt apart as you would for cleaning and

Briam - Have you by chance installed an aftermarket trigger group, or lighter weight springs in your factory trigger group? Have you installed a different firing pin? Is this with all types of ammo or just one type? Is this happening every round, or just
